4-H Judging Contract

I am curious, I recently took over our 4-H poultry club and have made arrangements to get a judge but I need to send him a contract. We have already agreed upon payment and everything, but I don't know what to send him. Does anyone have forms they use that they could e-mail me? I would really appreciate it.

I don't have our county's saved on my computer. You just need to type up a form with the name, date, time, place, type of judging, contact information, and the agreed on amount that is payable after the judging is complete. I fill out and sign the contract and send it to the judge to sign and send back.
